About this role:
You will initially be working closely with and receiving a high level of support from the manager and director
You will be working with NDIS participants who have psychosocial and other disabilities with complex support needs
You will be working closely with NDIS participants, their informal and formal supports to achieve outcomes guided by the person receiving support and / or their guardian
You will work to develop your knowledge of the various systems and legislations that may impact the people you support
You will be recognising and addressing complex barriers across multiple systems
You will be facilitating stakeholder collaboration
You may be transitioning people with severe psychosocial disabilities back into the community after long stays in institutional settings
You will be engaging in the development, monitoring, review and reporting of participants' NDIS plans and sharing progress with key stakeholders
You will be keeping accurate records and have an understanding of client confidentiality and the Privacy Act
You will need to work within the funding allocated, regularly monitor and track coordination billable hours
You will be mostly working in the Sydney area, but people you support may live in various locations across the greater Sydney area as well as hospitals and custodial settings in other locations in NSW.
Essential Criteria:
- Relevant tertiary qualifications in social work, health or social science, Allied Health, or Diploma in community work or equivalent and / or extensive experience in psychosocial support coordination
- Current National Police Check
- Permitted to work permanently with no restriction on hours
- Working with Children Check
- First Aid Certificate
- Current Driver's Licence
- NDIS working check
- Access to your own vehicle suitable for work purposes
- Minimum 1 – 2 years’ experience as a support coordinator
- Previous experience working with people with psychosocial disabilities
- Understanding of the Recovery Model, supported decision making, and risk enablement
- Excellent interpersonal skills, with the capacity to liaise effectively with a wide range of individuals and service providers
- Excellent report writing skills
- Computer literacy in word, excel, database, internet and email software
- Understanding of logistical, ethical and human rights issues affecting people with disability and their significant others
Desirable
- Previous experience in a case management or similar role
- Demonstrated experience coordinating multi-disciplinary teams to provide wrap around support
- Understanding of the NDIS Legislation and its relevance to support coordination
- Experience working with Health NSW and other Government bodies
