About the role:

This position of Reablement Support Worker works on a casual basis and closely with the Client Reablement Officer to plan, prepare and provide appropriate services for the frail aged, people with dementia and younger people with disabilities in accordance with the guidelines of the funding body and the City of Swan.

This position requires a person who can work as part of a dynamic team and must have a positive outlook when providing support to the members. We are looking for someone who is multi-skilled, able to identify changed responses in members and report concerns to the supervisor in a timely manner. The Reablement Support Worker will also be responsible for transporting members and providing clients with meaningful activities and outings so experience working in a day club and transporting members safely will be absolutely essential. 

Key Accountabilities include, but are not limited to:

  • In conjunction with the Client Reablement Officer, plan and facilitate efficient transportation for the Community Care Services clients
  • Support the development and implementation of Division’s Business Plan, projects and programs and the quality management principles.
  • Work collaboratively within a team environment to plan and implement programs for Community Care Services clients
  • Ensure that client meals are nutritious, well balanced, and cater for special dietary needs
  • Support a clean and safe environment for self and others
  • Perform other Club related duties

The successful candidate will have the following:

  • Experience in the provision of care and activity programming for frail aged, people with disabilities and/or people with dementia
  • Experience in manual handling of frail aged and people with a disability and/or people with dementia
  • Experience with driving and transporting members is essential
  • Experience in the provision of centre based respite
  • Working knowledge of the ageing process and/or the effects of dementia
  • Knowledge of Aged Care Quality Standards and NDIS Standards
  • Current National Police Clearance
  • Current Senior First Aid Certificate
  • LR class drivers licence with F endorsement
  • National Disability Screening Check
  • Covid and Flu Vaccination encouraged
  • Certificate III in service delivery for the aged, people with disabilities and/or people with dementia will be considered advantageous

About working with us:

The hourly rate for this casual position is from $31.12 - $33.37 depending on the skills and experience of the successful applicant, plus 25% casual loading.
