Job description

The Institute for Urban Indigenous Health (IUIH) leads the development, planning and delivery of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in Southeast Queensland. Together with our four Aboriginal community-controlled founding members, we are focused on our collective vision of healthy, strong and vibrant Aboriginal and Torres Strait Islander children, families and communities.

The Quality and Innovation team provides clinical oversight and support t0 IUIH member services and is responsible for the service development and delivery of urban Aboriginal and Torres Strait Islander health and family wellbeing care. This diverse team includes IUIH Data and Research, Logistics, Oral Health, Pharmacy, Policy, Social Health, and Regional Clinical Support.

About the role?

As a Regional Nurse Reliever, you will work across our IUIH and Member Services clinics where you will be responsible for specialist community, primary health nursing, outreach and Virtual Care services that focuses on the prevention, early detection and management of health problems for Aboriginal and Torres Strait Islander people. As a member of a multi-disciplinary team, the position ensures effective screening, service delivery and administration practices are delivered in accordance with the IUIH Model of Care.

Day to day duties will include:

  • Provide day to day health service activities within the context of Comprehensive Primary Health Care in a professional, confidential and culturally safe manner.
  •  Perform service delivery within scope of practice and within own personal experience and ability, observing all standard clinical practice policies and procedures.
  •  Provide routine screening and early detection of health conditions through initiation and performance of all aspects of comprehensive health assessments, other than areas identified as requiring the GP for completion.
  •  Provide testing, treatment and care under the direction of the GP including routine immunization, wound care, administration of prescribed medications, testing and monitoring procedures such as ECG, spirometry, BSL measurement, urinalysis, point-
  • of-care testing of Hb, HbA1C, ACR, etc., and other activities within recognized scope of practice.
  •  Initiate GPMPs, TCAs and their reviews, performing history, examination and relevant monitoring procedures and collating required information for GP completion.
  •  Enter any recalls and reminders issuing from the consultation into the electronic health record system.
  •  Provide follow-up monitoring and care as directed through health assessments and / or GPMP / TCAs, coordinating as required with other relevant internal, visiting, and external providers.
  •  Actively maintain knowledge of the application of the MBS relevant to your job role and ensure information about completed MBS items is effectively captured for accurate processing of claims.
  •  Attend to home visits where required and under the direction of the Practice Manager and / or Senior GPT provide health education and promotion on a one to one basis to all the services clients.
  •  Monitor and ensure maintenance of cold chain systems and other medical storage systems & clinical equipment.
  •  Ensure information is recorded accurately and in a timely manner in the electronic health record system, and that records are maintained in accordance with AGPAL standards.
  •  This role involves travelling to any IUIH member services across SEQ

About You? 

  • Current AHPRA Registration (Registered Nurse or Endorsed Enrolled Nurse)
  • Current C Class Drivers Licence (QLD)
  • Current CPR (willing to obtain)
  • Minimum Cert IV in Nursing qualification
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values and ways of doing business.
  • Demonstrated clinic based experience, in particular the ability to implement and support a medical/primary health care team and effectively plan and coordinate own and teams’ workload
  • Ability to work as part of a multi-disciplinary team and understand the cycle of care patient system
