What you'll be doing
As a care coordinator you will be the key contact to support individuals develop their recovery plan and personal goals using person centred and trauma informed approach to care.
People of Aboriginal and/or Torres Strait Islander background encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse – Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated effective interpersonal, verbal and written communication skills.
- Current mental health experience and knowledge and clinical problem solving ability.
- A commitment to excellence in mental health nursing practice and continuing professional development.
- Ability to work effectively within a multi-disciplinary team
- An understanding of the professional, ethical and legal requirements of the Registered Nurse.
- Demonstrated knowledge and skills in the use of information technology systems. Current NSW Driver licence.
