You will work independently and collaboratively in a multidisciplinary team utilising professional skills and ability to contribute to client centred care to meet the client’s individual health care needs, with the aim of maintaining and/or improving their health.Provide nursing assessment, care coordination, evidence based best practice clinical intervention, monitoring, maintenance and education to clients and their carers/families in the community.How to Apply

• Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

• Demonstrated ability to organise and prioritise work.
• Demonstrated effective communication skills both written and verbal. Effective interpersonal skills showing initiative and leadership.
• Demonstrated commitment to professional development, evidence-based practice, and an awareness of changes within the health care environment.
• Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
• Proven ability to work effectively and collaboratively within a multi-disciplinary team and ability to work independently.
• Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
• Current Drivers Licence valid within Australia and willingness to use for work purposes.
