Eastlakes Community Health Centre provides Community Nursing to clients in the East Lake Macquarie area.
About the job:

  • Community and Aged Care Services Greater Newcastle Sector Community Nursing is currently seeking a Registered Nurse
  • The position is for 3 days per fortnight, Thursday Friday, and Fridays alternate weeks.
  • In this role the Registered Nurse is responsible for the provision of person-centred; evidence-based Nursing Care that promotes positive health outcomes. Assess, plan, implement and evaluate Nursing Care in collaboration with individuals and the Multidisciplinary Healthcare Team. You will work in therapeutic and professional relationships with clients in their own homes. All staff are expected to act as an appropriate and effective role model and promote culture and supporting practices that reflect the NSW Health CORE values of Collaboration, Openness, Respect and Empowerment through demonstrated behaviors and interactions with patients, clients and employees.
  • An initial orientation period will be provided with support from mentors, Nurse Practitioners and Clinical Nurse Specialists. Ongoing professional development will be offered face-to-face and online.

Requirements:
Our successful candidate will possess the following:
  • Current registration as a Registered Nurse with AHPRA.
  • Recency of practice as a Registered Nurse in a Community Health setting would be highly regarded.
  • Strong organisational and communication skills, with a strong clinical background with experience in, but not limited to, wound assessment and management, urinary catheterisation including Supra Pubic catheter care, Central Venous access and maintenance.
Services provided include:
  • Wound assessment and clinical management
  • Generalist management of PICC Lines, Portacaths and Clexane.
  • Short term Diabetic follow-up post-change in medication regime.
  • Specific therapeutic medication management.
  • Palliative Care support in conjunction with other Palliative Care service providers.
  • General Nursing assessments and subsequent referral to other service providers.
  • Continence assessment and Clinical Management.
  • Urinary Catheter Care and Clinical Management.
