Job Description:
We are seeking a compassionate and skilled Community-Based Registered Nurse to join our dynamic team. As a Community-Based RN, you will provide high-quality nursing care to patients in their homes and within community settings. Your role will involve collaborating with a multidisciplinary team to support patients' health needs, promote wellness, and prevent illness through a holistic approach.
Key Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualised care plans.
- Administer medications, treatments, and procedures as prescribed.
- Monitor patients' progress and adjust care plans as needed.
- Educate patients and their families on managing chronic conditions, medications, and healthy lifestyle choices.
- Coordinate with other healthcare professionals, social services, and community resources to ensure holistic care.
- Maintain accurate and timely documentation of patient care and progress.
- Provide emotional support and guidance to patients and families.
- Participate in community health promotion and disease prevention programs.
- Current and valid Registered Nurse (RN) registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Bachelor of Nursing (BN) preferred.
- Minimum of 1 year of nursing experience, preferably in community health, home health, or a related field.
- Strong clinical skills with the ability to work independently and manage a diverse caseload.
- Excellent communication, interpersonal, and organisational skills.
- Compassionate and patient-centred approach to care.
- Current Australian driver's licence.
