Registered Nurse/ Case Manager

Acacia Living Group are looking for an experienced Registered Nurse to join the team as a Case Manager. We have multiple positions that would suit someone who is seeking a fulfilling, flexible role within the convenience of your local area. You’ll be supported by a passionate team of people dedicated to delivering excellent standards and quality of care. The roles are part time working from our Geraldton facility and includes some on-call duties.

As a Case Manager with Acacia Living Group, you’ll provide a range of clinical care and support services for Home Care clients, assisting them to experience a greater level of independence and ensuring their health and wellness are supported. Experience in case management is essential, and would suit someone who is innovative, client focused and highly motivated.
  
What you’ll do:

• Working independently with support from experienced senior clinicians
• Implementing and monitoring care of clients to enable them to live well at home
• Developing appropriate therapeutic and caring relationships
• Collaborating with health professionals to achieve client outcomes

To be successful, you will have:

• Previous experience as a Registered Nurse
• Relevant qualifications and AHPRA registration
• Valid working rights in Australia (Passport, Birth Certificate etc)
• Federal Police Check (valid within the last 3 years)
• Own vehicle and licence
• Evidence of up-to-date Flu Vaccination (or willingness to obtain)
• Evidence of up-to-date COVID-19 vaccinations

Please note -Geraldton is a regional coastal centre approximately 4 hours from Perth with limited accommodation available for rent and lease.  If you are applying from outside this region and don't have accommodation arranged or available to you, you may not be able to be considered for this role.
