We are currently recruiting for a Registered Nurse to work for a fantastic not for profit in the Inner West. This organisation supports older people who live in the local community who have home care packages. The role would entail carrying out assessments, understanding needs, managing budgets and assisting the person to live independently.
About the role
- Work with older people who have Home Care Packages
- Full Time position, with flexible work hours
- 9 day fortnight would be considered
- Work of the office and in the community
- Hybrid Work Arrangements (2 days per week from home)

- Salary of ~$82k plus super plus salary packaging - paid on the SCHADS award - Level 4.1

This organisation have been supporting people in the community for many years. They work specifically with people who come from Culturally and linguistically diverse backgrounds. You'd join an existing team and take on a manageable caseload.
Duties:
- Conduct initial assessment and prepare necessary care plans for clients.
- Case Management
- Formally review client care plans on an annual basis to ensure compliance to standards of care for quality review
- Monitor, evaluate and review care plans as required by client care needs
- Provide clinical advice and guidance to other team members
- Liaise with and refer to allied health, dementia and continence services and other medical practitioners as required
- Advise rostering team of client needs
- Ensure all services are addressing client needs according to care plan
- Networking with other services including ACAT and CHSP providers to ensure that the service is relevant to the needs of clients
- We are looking with previous experience working with older people
- It is an essential requirement to be an AHPRA Registered Nurse
