About the roleThe role as a Registered Nurse will involve delivering comprehensive holistic assessments and connected plus coordinated care to Aboriginal people, within our Integrated Planned Care for Better Health & ED to Community programs. This position operates under Western Virtual Service Delivery Directorate. If you have a career background in mental health, chronic disease or coordinating care for people with complex health needs then this may be the job for you.
The role will contribute to future service models and the provision of culturally safe and appropriate care for Aboriginal people across our communities in Western NSW. Engagement with key partners to improve experiences and outcomes for Aboriginal people living in our Local Health District, will also be a key focus of this position.
