Role Description
The District Nurse is a Registered Nurse with a broad level of professional nursing skill and community based knowledge from which they derive their practice.
The District Nurse partners with consumers and provides leadership to ensure that person-centred care is planned, implemented, evaluated and maintained at a high standard.
The District Nurse acts as a professional and clinical role model for staff in setting clinical standards and achieving high quality evidence-based nursing care.
The District Nursing team provides services to the community such as: Wound management, medication monitoring/education, continence and catheter care, palliative care, general health monitoring, health assessments and referrals, hygiene assessments, advanced care planning.
