Specific Accountabilities
Specific accountabilities are the outcomes that you are required to produce. They describe why your job exists.
- Ensures that all clients are respected and treated with empathy and dignity through providing them with choice to live their daily life in a way that is special and unique to them and that is consistent with their wellbeing and the wellbeing of others.
- Ensures a holistic approach to client care by providing intake assessments, nursing assessments, brief intervention and clinical reviews as well as acting as a professional resource for GPs and Primary Health Care practitioners working with clients with the relevant medical needs.
- Ensures documentation meets organisational, legislative and funding requirements by completing and maintaining appropriate client documentation and statistical information and sharing them with the relevant stakeholders to ensure timely completion of duties.
- Ensures ongoing professional development by attending regular clinical supervision, relevant training, forums, courses and other professional development activities as agreed with manager.
- Contributes to the effective functioning of the team by sharing clinical knowledge and skills and informing team members about clinical changes.
- Contributes to the team achievement of client program goals and outcomes by conducting comprehensive assessment, planning, implementation and evaluation of care in collaboration with the client and the multidisciplinary team.
- Contributes to the development of a multi-disciplinary approach by providing support to other clinicians through peer supervision.
- Contributes to recovery plans by being involved in developing, reviewing, discussing and writing consumer care plans.
- Contributes to the continuous improvement of the program by assisting with policies, procedures and work practices.
- Contributes to an understanding and respectful work environment and work practices, by valuing all people and giving rightful consideration of people's cultural backgrounds, including Aboriginal and Torres Strait Islander people as Australia's first people.
- Ensures that each client's overall health(both physical and mental) is maintained at the highest possible level by providing routine (injections, wound care and so on) and non-routine (illnesses, falls and so on) clinical care in accordance with the client's needs, safety requirements, and clinical guidelines and informing the doctors when further assistance is required.
- Ensures the relevant families of the person being cared for are fully informed about issues and changes through timely communication using different communication methods such as reports, verbal and being involved in case conferences.
- Contributes to the safety culture by keeping the workplace safe, clean and tidy, complying always with safe work practices, participating in workplace consultation activities, actively participating in safety activities, including hazard and incident reporting as prescribed in the WHSIMS.
Education and Experience
- Registered Nurse (Full registration with AHPRA).
- Previous experience in aged care is desirable.
