The OpportunityThis permanent part-time (0.6FTE) position will monitor client progress and evaluate care plans within High Risk Foot Clinic (HRFC) and the Health Independence Programs.Key responsibilities: Provide clinical support to Podiatrist and clients attending the High Risk Foot Clinic;Care Coordination for clients under the Health Independence Program;Coordinate sterilising of instruments, stock management and ordering of product;Manage diary bookings including day to day appointments and those on a waiting list;Participate in multi-disciplinary case conferencing, including the preparation of agendas, chairing the meeting, completing minutes and follow up;Provide education and learning opportunities for staff at Benalla Health;Participate in Quality Improvement activitiesProvide assistance to Intake Clinician for clients referred to Podiatry and High Risk Foot Clinic.Key Selection Criteria:Achievement of competencies for a registered professional, as prescribed by the Australian Health Practitioner Regulation Agency (AHPRA)Experience in Chronic Disease Management and/or multidisciplinary team care programs in a community setting (desirable);Proven ability to work effectively within a multidisciplinary environment with the ability to manage time and conflicting priorities;Well-developed communication and interpersonal skills; Why choose Benalla Health?
