About the job:

  • Are you a passionate and driven Aboriginal or Torres Strait Islander Registered Nurse who is looking for the chance to directly influence the health of your Mob and other Aboriginal communities?
  • How you ask? As an ICCAPP RN you will have the opportunity to utilise your many clinical skills to focus on preventative, self-empowering health strategies for your clients, their families and the broader community.
  • This opportunity provides for the perfect chance to “test the waters” in the community space.
  • The ICCAPP Team sits within the broader Referral Intake and Planned Care for Better Health Teams, operationally managed by a Nurse Manager.

  • You will not work alone, the ICCAPP Team has a Clinical Nurse Consultant and Registered Nurses, who can’t wait to meet you and share their experiences and knowledge to help you settle into your new role. In addition there are also teams with expert knowledge in navigating the community services, who are also here to support you.
  • No day is ever the same, as you can provide a range of services including, Clinical Assessment, Health Coaching, Case Co-ordination, Health Screening, 48-hour follow-up phone support and yarning to Aboriginal people and their community. This promotes health literacy and guides in the improvement in health outcomes. This can happen in homes, community, clinics, coffee shops or telehealth. The approach is client centred, flexible and non-judgmental.

Requirements:

Our successful candidate will possess the following:

  • Current registration as a Registered Nurse with AHPRA.
  • Current NSW Drivers Licence.
  • Passion for working within the Aboriginal and Torres Strait Islander community to actively influence positive health outcomes and great communication skills.
  • Enthusiastic, love a challenge, positive can-do attitude, innovative out of the box thinker, looking for a change.
  • A background in chronic diseases or care co-ordination/navigation is great but not essential.
