Astley Care is an innovative not-for-profit community based aged care provider, providing quality person-centred care in South East Metropolitan Perth. Reporting to the Clinical Care Manager, this role will provide services to our higher care clients.
This role is available on a permanent 0.6 FTE part-time basis.
Role and responsibilities:
- General nursing duties including wound, catheter, pressure care and dressings
- Assisting with diabetic management, bowel management and monitoring nutritional intake
- Providing Nursing Assessments for high care clients and continually monitoring their conditions and education
- Conducting Psychogeriatric Assessment Scale (PAS) assessments of clients with dementia.
- Liaising with other health care professionals as required and advocating for our clients
- Ensuring all procedures and case notes are documented in line with agency policy
Qualifications and Requirements:
- Bachelor of Nursing, Registered with AHPRA as a Registered Nurse
- C class WA driving license and a clean, roadworthy vehicle
- Current National Police Clearance within last 6 months
- Current Covid-19 vaccinations
- Current First Aid Certificate
- Pre-employment health assessment (no cost) aligned to the inherent requirements of the role for work health and safety purposes
Skills and Experience
- Minimum 2 years nursing experience, preferably working with aged people living in the community
- Experienced in wound care assessment and management, catheter care, knowledge of best practice standards in these
- Demonstrated understanding of the Aged Care Quality Standards
- Commitment to embrace Astley Care's vision to provide Choice, Dignity and Independence to seniors and carers in our community
- Highly developed interpersonal and communication skills.
- Solid time management, organisation and prioritisation skills. Meets deadlines and delivers agreed objectives by prioritising and managing tasks through to completion
