Job description

As part of the Rehab ward, you will be accountable for the provision of safe and holistic care to individuals and their families, applying best clinical practice and ensuring a safe environment for optimal patient outcomes. You’ll deliver exceptional person-centred care by working collaboratively with your multidisciplinary team, applying exceptional communication and customer service skills.

Your part in our UnitingCare team

- Assessing, planning and implementing client care within your scope
- Considering physical, social and psychological requirements
- Creating and updating care plans to reflect developing needs
- Ensuring clinical quality compliance standards are met
- Working collaboratively with other multi-disciplinary team members.

What makes a difference for us?

  • Minimum of 12 months’ experience in an acute care/hospital setting
  • Previous rehab experience 
  • Bachelor of nursing degree or equivalent and AHPRA registration
  • Commitment to patient safety and quality improvements
  • Desire for ongoing professional development
  • Availability to work across a 24/7 fortnightly rotating roster
