About the program
The Residential in Reach program aims to prevent avoidable transfer to the Emergency Department for residents in Aged Care facilities. The Program provides assessment/intervention when the regular general practitioner is unavailable, the resident requires medical attention and is at risk of transfer to ED. The program is a team of clinicians, including Geriatrician, Nurse Practitioner, roster GP's and Geriatric Medicine Registrar.
About the role
The Role is responsible for providing assessment, triage and coordinated care to residents in aged care facilities with a multidisciplinary team approach. Work will include direct client care, and secondary consult supporting staff within the team and other services including GP's.
