The successful applicant must be available to work Wednesdays. The successful applicant must be willing to complete Advanced Nurse Practice certificate within 24 months, all training provided free of charge via Tresillian.What you'll be doingThe Child and Family Health Nurse (CFHN) in SNF practices at an extended level of nursing, working within a primary healthcare model with families who are identified as a part of the SAFESTART process as having level 2 vulnerabilities. Care will commence wherever possible in the antenatal period or up to 4 weeks in the postnatal period. Mothers and their infants are visited following a structured schedule of visits up until the baby reaches 2 years of age.The CFHN will work as a part of a multidisciplinary team in both primary and secondary services.
The CFHN will provide comprehensive Child and Family Health Nursing, including but not limited to home visits, clinic, group work and intake.There may be a requirement for the CFHN to work in universal & or targeted Child and Family Health nursing services.
The position involves working with families with complex issues and the potential exposure to trauma.

- Relevant experience with Sustaining NSW Families and/or willingness to complete the Advanced Nurse Practice course within 12 months of employment (through Tresillian).
- Demonstrated ability to organise and prioritise work.
- Demonstrated effective interpersonal and communication skills both written and verbal.
- Evidence of post registration experience working with families and young children; including those from diverse cultural and linguistic backgrounds using a family partnership model.
- Demonstrated understanding of the current role of the Child and Family Health nurse, including the principles of primary health care and the NSW Health Supporting Families Early Maternal and Child Health Home Visiting policy.
- Demonstrated ability and experience working in a multidisciplinary team.
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and a commitment to undertaking training in electronic health record systems.
