Albury Wodonga Aboriginal Health Service (AWAHS) is a highly regarded not for profit community medical practice delivering services to local First Nations people. They have a great reputation for providing patients with high quality holistic healthcare services over many years on the Border.
From their modern busy medical centre in Glenroy, they deliver a range of primary health services including Chronic Disease management, sexual health, wound care, immunisations, diabetes education and more.
RN Practice Nurse / Duty Officer- Three days a week (casual contract initially)
- Albury based - and Wodonga when required
- $50/hr + superannuation + excellent salary packaging tax benefits
We are now looking for a part time Registered Nurse with the following skillsets and experience:
- Triaging, general health screening and assessments
- Competent in coordinating GP waitlists
- Experience in Chronic Disease management
- A passion for working with diverse patients in a genuinely caring environment
This key position is offered as part time at a casual rate of $50 per hour plus superannuation, for the right candidate. Salary Packaging is also available to maximise take home pay.
Chronic Disease RN - part time- Permanent Part Time - 2 days per week
- $44 p/hour++ superannuation + excellent salary packaging tax benefits
We also have a great opportunity for a part time Chronic Disease Nurse to work Mondays and Tuesdays (preferred).
We seek a Registered Nurse who is passionate about quality care delivery and patient education surrounding chronic disease management.
The role of the Chronic Disease Nurse is to work with clients that have been identified as having a chronic disease; including working closely with AWAHS GPs, Practice Nurses and Aboriginal Health Practitioners around the preparation of chronic disease management plans and health checks, involvement in disease specific clinics, referrals to allied health services, referrals to health programs, and regular review of recall register.
If you have an interest in health promotion activities designed to promote community wellbeing through health information, promotional brochures, community development & self-care initiatives, then this is the job for you.
You will ideally have experience working in chronic disease management, developing and managing chronic disease care plans and running effective chronic disease clinics.
