Form of workYour Lifestyle Centre @Beenleigh is a Not For Profit Senior Citizens Centre based in the heart of Beenleigh. We have numerous activities, programs and Meals on Wheels running from the Centre.
The role of the Registered/Enrolled Nurse Case Manager is responsible for the care management of clients receiving aged care services at home and in the community. You will be required to manage your own case load, undergo clinical assessments for participants and maintain accurate client records through case noting. This role will see you build and maintain positive relationships with key stakeholders including service delivery partners, relevant government assessment agencies and health professionals.
As an integral member of a multi-disciplinary team, you will have a strong emphasis on holistic care with a wellness focus for the frail aged and people with dementia. Utilising your exceptional clinical and people skills, you will have a caring and empathetic disposition.
Essential Criteria
- Current and valid Driver’s License
- Cleared National Criminal History Check,(police check) less than 6 months old, or willing to obtain.
- Knowledge of Aged Care Services, Aged Care Quality standards, community resources and service networks for older Australians and their carers.
- Must have a Bachelor of Nursing, registered with AHPRA or Enrolled Nurse Diploma.
- Experience in conducting Home Aged Care assessments for clients needing more services.
- Understanding of Aged Care Sector, Home Care Packages/CHSP, Aged Care Quality Standards, Code of Conduct, and legislative frameworks.
- Strong computer literacy and ability to use Microsoft Office packages.
- Legal right to work in Australia with no restrictions is a necessity.
