Registered Nurse/ Case Manager –

We currently have a vacancy for a Registered Nurse /Case Manager to join our growing team in our Camp Hill office. This position would be suited to an enthusiastic leader, who thrives in a collaborative working environment to ensure that the best service is provided to our participants.

Salary would be negotiable within the salary bracket according to experience and knowledge.
Role Inclusions:
- All-encompassing case management services from responding to an initial referral, signing service agreements, developing and monitoring individual plans and budgets with regular reviews, recruiting appropriate staff and allocating services for your client base.
- Consulting with participants, carers, and families to facilitate care assessments, planning, and reviews.
- Ensure that all services are effectively implemented and managed in line with the Care Plan.
- Working with legislated NDIS and Consumer Directed Care frameworks.
- Developing relationships with families and carers that acknowledge their needs
- Working alongside our participants to set individual goals that are realistic and achievable
- The position will support participants in Brisbane/Logan and surrounding areas
- Provide clinical advice and training to support staff
- Provide clinical review of participants where necessary
To be considered for this role you will need:
- Qualification in Nursing with current registration
- Sound clinical knowledge and understanding
- Experience and good knowledge of the NDIS
- Experience and good knowledge of My Aged Care (Home Care Packages)
- Outstanding customer service skills and the ability to build good rapport
- NDIS Support Coordination knowledge/ experience would be great but not required
- A reliable vehicle and current unrestricted Driver’s Licence
