The Relief Support Worker – Aged Care position is designed to seamlessly maintain continuity of services, providing coverage during short term absences & vacant shifts. This role helps our clients to remain independent & in their own homes for as long as possible by following client support plans, assisting with daily living & providing general cleaning. Additionally, the Relief Support Worker helps our scheduling team by making phone calls & assisting with onboarding new clients.
A typical day might look like…
- Travel to various client’s homes to fill vacant services across Rise’s Aged Care division.
- Follow client support plans to assist with daily living.
- Follow medication assistance procedure.
- Assist clients to achieve their individual goals.
- Record & report client changes to coordinators.
- Contribute to program meetings.
- Assist Rise’s scheduling team with making phone calls.
This role is full-time, working Monday to Friday from 10.00am – 6.00pm. You will be required to travel to Midland & surrounds, Mundaring & surrounds, Kalamunda & surrounds & Forrestfield & surrounds.

- Certificate III in Community Services and/or related qualification with experience in aged care support work.
- Medication trained ideally with practical hands-on experience.
- Demonstrated ability to follow routines & procedures.
- Ability to bend, kneel and lift correctly to complete duties safely.

Appointments to this position will be subject to a National Police Clearance / requires a driving licence and reliable vehicle to use for work purposes / requires a pre-employment medical assessment.
Costs for new checks will be covered and/or reimbursed by Rise.
