👉🏼 The need-to-know…

As a Community Relief Support Worker, you will support people with disability, families and carers living in the community to live an enriched life and develop skills and capacity to promote independence in their homes and community. Through a person-centred approach, you will support people to identify and develop their goals and achieve their unique potential.

A typical day in the role may look like… 

  • Work alongside people you support to achieve goals and aspirations they have identified in their individualised plan.
  • Support people using a combination of person centred and capacity building approaches in response to their needs.
  • Support clients to explore, engage and build upon existing interests to enhance their social and economic participation.
  • Assist and work alongside the people/s you support with elements of their daily living, documented in the support plans.
  • Increase a person’s independence by supporting their decision making and problem solving.
  • Use client management systems to accurately record visit details and client progress, outcomes, measurement tools, risk assessments and document dated notes.
  • Support with the induction of new workers through buddy shifts and sharing of information.

This role is a permanent, full-time position working Monday to Friday. You will be required to use your own vehicle to drive around Perth Metro areas to meet clients. You will be reimbursed for travel time & kilometres between clients and during a service. 

We have three shift times to fill, please indicate within your application which shift time is your preference.

Shift One: Monday to Friday, 7.30am – 3.30pm.

Shift Two: Monday to Friday, 8.30am – 4.30pm.

Shift Three: Monday to Friday, 9.30am – 5.30pm.

👉🏼 The need-to-have…

Here at Rise, we nurture potential. But we do need you to have a few tricks up your sleeve including the following:

  • A qualification in Human Services e.g. Disability, Aged Care, Allied Health, Community Services.
  • Experience with providing positive behaviour support.
  • Experience in the same or a similar role working with people with a Disability of all ages and backgrounds.
  • Demonstrated ability to work autonomously and within a diverse team.
  • Demonstrated ability to use initiative, discernment and judgement while maintaining professional boundaries.
  • Well-developed positive communication skills within a variety of settings (written and verbal).
  • Demonstrated understanding and knowledge of policies, practices, and statutory requirements of work within Disability sector.

Appointments to this position will be subject to a NDIS Worker Screening Check / requires a driving licence and reliable vehicle to use for work purposes / First Aid Certificate.

Costs for new checks will be covered and/or reimbursed by Rise.
