The Residential In-reach (RIR) Team Leader will provide operational and clinical leadership to the RIR team ensuring delivery of an effective and responsive service. The team leader will manage the day to day clinical operations of the service and will monitor and support team activity. The RIR team leader will assist the manager in recruitment, staff orientation, staff rostering and staff performance planning and will provide clinical and professional advice and supervision to the team, lead case conference and other team specific meetings, and ensure the team operates within accepted best practice guidelines.
The team leader will work closely and foster strong relationship with medical staff from Geriatric Medical team. The Team Leader will also develop and deliver educational programs to RIR staff as well as on occasion to staff of aged care facilities. The RIR team leader will also be responsible for the RIR quality improvement projects and auditing.
Coordinate the day to day operation of the service, including rostering, managing planned and unplanned absences, monitoring of staff movement and safety, coordinating the day to day management of medical staff, coordinating and leading team meetings and case conference
Monitor and ensure activity targets are met.
Provide clinical supervision to staff.
Develop, implement and evaluate services and projects/ initiatives to ensure effectiveness and improve service delivery of the team/stream.
Act as a resource for other departments in clinical matters or in relation to RIR initiatives.
Maintain an excellent working knowledge of current treatment options and pursue learning in the field of clinical best practice in aged care nursing management, providing expert clinical support to other staff and ensuring all care is underpinned by evidence-based practice
Triage phone referrals to ensure care is provided in the most appropriate setting.
Provide expert clinical advice to patients, carers and other health care professionals within the defined speciality
Provide specialist assessment and management interventions for RACF residents referred to the RIR Service, by phone, telehealth and face to face within the RACF setting, the Emergency Department or other venue as required. This may also include limited inpatient consultations.
Classifications for this position will range from classification range ZA7 - ZA8 ($115,850 - $118,606 per annum, pro rata).
Your Contribution
Diploma/Degree in Registered Nursing Division 1
Minimum of seven (7) years’ post-graduate experience.
Minimum three (3) years’ experience in senior or management levels in the area of Aged Care and/or Emergency nursing, acute care nursing
Demonstrated advanced skills in comprehensive assessment of the older person
Demonstrated ability to both lead and support an interdisciplinary team and to also function as an autonomous practitioner
A demonstrated knowledge of current evidence based practices in the area of Aged Care assessment and management, including of issues that affect aged health in the hospital/community interface
Relevant qualifications in Aged Care and/or Emergency nursing
Current AHPRA Nursing Registration
Clinical expertise in Aged Care, Emergency nursing and or acute care nursing,
Current Driver’s Licence
Current National criminal history check, or willing to obtain

Health Independence Program
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.
This position will be part of the HIP Residential Aged Care In-Reach Service (RIR). The key aim of the RIR is to provide hospital type care, where appropriate and safe, to people living in aged care facilities (RACF) as an alternative to an emergency department presentation. The RIR is staffed by Clinical Nurse Consultants and doctors from the Geriatric Medical team. RIR is a 7 day per week nurse lead service.
