Qualifications & experience

• Relevant industry qualifications for an EN/RN
• Registration with AHPRA - current licence to practice
• Experience working in a primary health care setting
• Experience working with patients with a chronic disease and preparing care plans
• Ability to work independently and as part of a wider team
• Excellent communication skills

Tasks & responsibilities

• Working in the treatment room assisting with the acute and ongoing care of our patients, i.e. wound care, vaccinations, CDM, care plans and minor procedures
• Able to demonstrate comprehensive and accurate knowledge and skills in providing episodic and ongoing care that is responsive to individual and group circumstances and environments
• Initiate and conduct comprehensive health maintenance, diagnostic activities and health promotion in partnership with individuals, groups and the GP team
• Provide evidence based information, resources and education to assist individuals, groups and families to make health care decisions
• Demonstrate proficiency in the use of information management technology and systems to inform clinical care management
