The Opportunity
We have permanent full-time or part-time positions available with flexible roster options. These roles are open to both graduate and experienced RNs & ENs.
The role of the RN/EN Case Manager is to provide a range of clinical care and case management services, designed to assist the care recipients experience a greater level of independence and ensure their health and wellness are supported.
The successful applicants will need to demonstrate sound clinical skills, be consumer-focused, a good understanding of aged care accreditation processes and an appreciation of current and future aged care funding models.
Key responsibilities of the role:
- Assessing, planning and implementing a range of holistic support and home care services
- Manage a diverse case load with dignity, respect and confidentiality
- Complete initial assessments for home care clients
- Develop and evaluate client directed, goal-based support plans incorporating a wellness focus for Home Care Package clients
What you’ll need:
- Relevant qualifications and current AHPRA registration
- Knowledge of Aged Care Quality Standards (or the ability to acquire this)
- Satisfactory national police check within the last three months
- Tasmanian Working with Vulnerable People registration with NDIS endorsement
- If you are on a visa, a copy of your current IMMI Letter
