You are a passionate and caring for ageing Australians in their homes and the community in which we live and want to showcase your skills in a supportive environment with an exceptional team culture.
You have a well-developed knowledge of the Aged Care sector, and the related Aged Care standards in the Home and Community setting where you have already spent a number of years.
You work pragmatically in managing complex clients who are receiving Home Care Packages, and structure your days and times to effectively and efficiently achieve client care and support outcomes within clients funded/financial capacity.
About the Role
As part of a Multidisciplinary Team, St Basil's RNs and Allied Health Practitioners spend their days engaging with and supporting the needs and preferences of our communities ageing population, to see them achieve their goals while remaining safely and independently in their homes.
Working alongside side our Senior Care Coordinator and Care Teams, the MDT support one another to holistically oversee all of the clients clinical requirements, manage their clinical care and ensure their needs are met, while remaining mindful of the clients funding and budgets, and regularly review and reassess clients to ensure their safety and wellbeing is appropriately prioritised.
Our MDT members operate in a mobile role that is out in our clients homes and engages and collaborates with the rest of our team while in the office for the clients care needs.
Your skills will include:
- At least 2 years experience in as an RN/Allied Health Practitioner (OT, Physio).
- Experience in client care in the Home Care setting, including conducting assessments, developing reports, recommendations and care plans, provision of care in the Home and engaging additional MDT as required.
- Providing great Client/Family/Carer Customer Service.
- Demonstrated ability to plan, organise and prioritise workload to meet tight deadlines.
- Ability to identify and escalate a clients changing health status.
- Demonstrated empathy, understanding, and experience of working with people living with dementia.
- High standard of verbal, interpersonal and written communication skills that facilitate positive outcomes for all.
- Positive work commitment, honesty and integrity.
- Ability to work independently and as part of a team.
- Skilled in providing support to aged persons assessed as requiring home care support.
- Use of client management software.
- Computer literacy in Microsoft Office.
- Customer relationship management - staff and client calls.
- Diligence when developing and writing client comprehensive clinical assessments, reviews and care plans
- Identifying & responding to customer's changing needs.
- Dementia care and the management of associated challenges.
- Work closely with the multidisciplinary teams to ensure all clients receive timely and appropriate clinical care and services.
- Other skills, e.g. strength and mobility, continence management and support; chronic disease management; stoma care; medication management.
Your qualifications / registrations will include:
- Current registration with relevant authority/body
- Qualifications/ experience in other specialties; including Aged Care related areas such as Mobility, Pressure Injury Management, Strength and Rehabilitation Management etc would be highly advantageous.
- Experience working in a Home and Community environment.
- Current first aid certificate - highly regarded.
- National Police Check – current within 1 year
- Current driver’s license and functional vehicle
- Recent Flu Vaccination (2024).
- COVID19 Vaccinations (min 3).
