About the Role
Carealot is seeking a dedicated and experienced Care Partner - Clinical to join our friendly and supportive team based in our Albany Office. The role will initially be Part-time (Mon, Thru and Fri), from 8.00 am - 4.00 pm.
As the Care partner, you are responsible for assessing, planning, implementing, and evaluating care for and with Carealot consumers. This position delivers inclusive and person-centred quality care and support in accordance with individualised consumer Care Plans to ensure optimal service provision that actively promotes choice, independence, and quality of life in a safe and home-like environment.
The role is responsible for fulfilling the following duties but not limited to:
- Ensuring effective planning, development and delivery of high-quality and sustainable services.
- Ensuring services are managed by funding requirements, policies, allocated budgets, and human resource management approaches.
- Developing effective systems to ensure compliance with quality and risk management accountability requirements and reporting processes in accordance with funding requirements.
- Leading and developing the team to identify sustainable service development and improvements.
Essential Criteria:
Experience and knowledge in Aged and Home and Community Care.
Current Enrolled or Registered Nurse registration (APHRA) is highly desirable.- Excellent written, verbal, and interpersonal communication skills that build effective leadership, develops inclusive teamwork, and establishes successful working relationships with stakeholders.
- Customer-focused work ethic, strong commitment to Carealot's Mission and values
- Strong person-centered skills and behaviours in communicating and engaging with consumers with dementia and/or high clinical/other support needs.
- Demonstrated ability to successfully assess, plan, implement and evaluate client care. This includes a strong commitment to providing safe, high-quality care.
- Strong analytical and problem-solving skills, with the ability to organise and prioritise tasks, work effectively under pressure, meet deadlines, and use resources effectively, safely, and efficiently.
- Demonstrated knowledge and understanding of continuous quality improvement, quality standards and the accreditation processes.
- Demonstrated ability to effectively manage change in a care related environment in line with operational and strategic goals.
- Current satisfactory National Police Certificate (obtained within the last 3 months).
- Working knowledge of safety and health legislation, the Aged Care Act (1997), and the Aged Care Quality Standards (desirable).
