Job description

Are you a compassionate and experienced Health Coach with a passion for working with older adults? Do you have a strong background developing wellness plans and supporting individuals in achieving their health goals? If so, we are seeking a Senior Health Coach for our 8-week Short- Term Restorative program for older adults.

As a Senior Health Coach, you will partner with and advocate for clients to improve the quality of their care across an 8-week program. You will act as an advocate empowering clients and their families/carers to improve health literacy. Through coaching, education, and use of assistive technologies you will support clients to achieve and maintain optimum levels for health and wellbeing. 

In this role, your exceptional organisational and administrative skills will be essential. you will be at the forefront of care management and coordination, working closely with clients and multidisciplinary teams.  As a dedicated health professional committed to person-centred care, you may be seeking a fresh avenue to leverage your expertise. This is an opportunity to collaborate with fellow health professionals in implementing restorative or reablement activities for our clients, making a meaningful difference in their lives.

  • You will be visiting our clients in their homes to assess, plan, implement and coordinate their Short-Term Restorative Care program in the Townsville region.
  • You will work within Aged Care Standards
  • You will monitor the Health Management of clients over an 8-week program.
  • You will empower and support clients to achieve their optimal levels of functioning independence and self-care.
  • You will educate clients, carers and their families around the areas of wellness and reablement including ongoing Chronic Disease Management
  • You will enjoy working independently to promote the service and to seek referral of new clients to ensure full occupancy of the program.
  • You will be working with a multidisciplinary team. Including working with GP's
  • Building and maintain relationships with local health professionals and services to ensure a high quality of care is maintained throughout the program.

What you need to be considered for the role?

  • Degree qualification in nursing or allied health
  • Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
  • 3+ years of clinical or health management experience in nursing or allied health
  • Experience within a health or community aged care setting and ongoing commitment to Continued Professional Development and Education
  • Provision of services in consumer directed care framework including experience of community nursing, or allied health therapy.
  • Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion
  • Demonstrated understanding of evidenced based care.
  • Ability to conduct Case Management and Health assessments. 
  • Development of care plans built in collaboration with clients and other health professionals.
  • Ability to work as part of a wider team and autonomously in a remote model of working.
  • Ability to organise work in a systematic, timely and accurate manner.
  • Proficient use of technology, including mobile platforms and telehealth
  • Current driver's license and ability to travel within the region on a regular basis.
