Job description

The purpose of this role is to provide assistance to ensure day-to-day efficiency with the overall aim of achieving business objectives. You will be required to assist the service function of the business in achieving operational effectiveness in line with business requirements. You will provide support in participants homes and the community to increase their individual skills, help build their capacity and achieve their goals. You will promote and provide environments that support the safety, independence and growth, health and well-being of our participants and staff. You will be the first point of contact for your allocated group of disability workers. You will provide leadership and support for your team to provide the best services they can to our participants.
Main Tasks:
Coordinate, plan and execute workflow activities.
Coordinate service personnel to ensure daily targets and tasks are completed.
Ensure workplace materials and documentation are maintained in line with legal obligations and best practice standards.
Regularly review and implement changes to various forms of documentation.
Assist in the management of third-party communication and technology queries and networking.
Ensure business operational needs are met · Plan, Prioritize, meet deadlines and competing demands, while working in a fast-paced environment.
Regularly review and identify changes to various forms of documentation etc. NDIS plans, Notes, Participant Goals and make sure all members of your team are completing the appropriate documentation.
Contribute to the continuous improvement culture that reviews indicators or client satisfaction to achieve results.
Assist in the management of third-party communication and technology queries and networking to ensure business operational needs are met.
Monitoring and supporting your allocated team of disability workers within their roles.
Developing and encouraging professional growth in your team of support workers.
Allocating and defining tasks for your team to perform.
Support participants in age-appropriate activities and tasks, in SIL, Respite, in home support and community access.
Provide direct care assistance to each participant in your allocated group in accordance with their service plan and as directed by the service delivery and engagement manager.
Work in accordance with the medication administration policy, work health and safety procedures and participants rights and policies, including privacy and confidentiality.
Monitor the safety and well-being of the participants and report any concerns to the senior management team.
Provide 1:1 or small group training and support in response to individual support needs.
Participate in planning and working with participants, family/carer, case managers and any other professionals to support the participants individual goals.
Participate in Performance Development to identify ongoing training and support opportunities for your Team of support workers.
Collaborate with rostering to maintain and ensure participants supports are being met.
Undertake on call duties as per the on call policies and procedures always including remaining contactable via phone whilst on-call, including outside the normal business hours of operation, and not being under the influence or any other substance which may impact your duties. Working with the WHS/Staff Liaison Officer to identify and reduce risks.
