As a Social Care Coordinator, you will be responsible for coordinating and implementing Social Care programs and services for our clients. You will work closely with clients, their families, and other healthcare professionals to ensure the highest level of care and support.In this role, you will assess client needs, develop care plans, and monitor the progress of clients throughout their care journey. You will also be responsible for coordinating with external healthcare providers and community organizations to ensure seamless care transitions and access to necessary resources.The ideal candidate for this position is empathetic, organized, and has a strong understanding of Social Care systems and resources. They should have excellent communication and interpersonal skills to effectively collaborate with clients, families, and healthcare professionals.
