💼 Let’s talk about the role:
As a Social Support Activities Facilitator, you will play a crucial role in promoting active community involvement for individuals eligible for the program through the Commonwealth Home Support Program (CHSP) or Home Care Package funding. This includes the frail, aged, or those with disabilities. You'll ensure the safety and wellbeing of participants in their chosen activities, supporting our mission to provide innovative and flexible program delivery.
To be successful in this role, you will have:
✅Certificate III in Individual Support (Disability, Home, and Community Care and /or Aged Care) or similar certificate III in Allied Health.
✅Current First Aid Certificate (in the last 3 years)
✅Current CPR Certificate (in the last 12 months)
✅A current full driver's licence and confidence to drive a 12-seat minibus in both rural and metropolitan locations
✅Fully vaccinated for COVID-19 and yearly influenza.
