A little about the role
As a Social Support Worker, you will be responsible for:
- Delivering group programs onsite at DPV Health’s range of locations and in the community.
- Ensure supports delivered are consumer directed approach as outlined in DPV Health Excellent Care Framework and Your Health Care Your Way guidelines.
- Contributing to assessments of the client’s goals, interests and needs.
- Assisting the Team Leader to maintain appropriate communication with carers and family members.
- Provide personal care to clients in the program, including prompting, toileting, and dressing.
- Maintaining accurate comprehensive case notes and undertaking administrative duties.
About you
To be considered for this role, you will need the following:
- Certificate III in Aged Care or Disability or Individual Support, or currently working towards a relevant health qualification
- Understanding of what is involved in working with people who are ageing and /or with a disability / mental health
- Good communication and negotiation skills, with the ability to build rapport with clients and families
- Excellent time management, problem solving, communication and computer skills
- Ability to work independently and in a team to create a positive culture
- Demonstrated knowledge and understanding of the Aged Care Quality Standards, NDIS Code of Conduct and NDIS Quality and Safeguards Commission, Active Service Model
- Community second language – Strongly preferred

- Unrestricted Victorian Drivers Licence
- Experience and ability to drive DPV Health Buses (11-seater)
- This position will require some weekend work/overnight stays
- Level 2 First Aid Certificate
