When We Need You:
- Part-time 3 - 4 days per week, 8am to 4.30pm
About the Role
We are looking for a Social Worker to join our multidisciplinary team:
- To provide counselling and support to our patients and their families/carers
- To co-ordinate appropriate and timely provision of community services deemed necessary for safe discharge
- To meet with the patient and carer throughout the hospital stay to ensure that their needs and concerns have been recognised and addressed
- To complete an accurate discharge plan and document
- To establish and maintain an association with community services
- To provide education and current healthcare information to patients and carers
What We Need:
- Right to work in Australia
- Registration with the relevant professional body
- High level of communication, interpersonal and customer service skills
- Demonstrated effective time management skills and ability to prioritise competing workloads
- Ability to remain calm in stressful situations and challenging circumstances
- Ability to work both independently and as part of a team
- Ability to identify and understand customer needs and offer interventions or problem-solving strategies
- Experience in discharge planning
- Satisfactory Police Check
- Evidence of protection against specified infectious diseases, including COVID-19
