Social Worker HP4 - Sunnybank Private Hospital (Casual)
Provide supportive and coordinated guidance, both clinically and administratively to patients across Inpatients and Outpatient Day Therapy Programs.
We are looking for a experienced hospital Social Worker with passion for excellence in customer service delivery to join our team.
As a Social Worker your responsibilities will include:
Working independently with the patients and as a multidisciplinary team member. Providing Social Worker support to patients across the Inpatient Medical, Surgical, Rehabilitation and CCU wards. Working closely with other members of the Allied Health Team including Discharge Planners to facilitate successful safe transition from hospital to home. Liaising with local support providers to meet the needs of patients discharging from hospital.

Sunnybank Private Hospital is a 122 bed facility located on the Southside of Brisbane, offering acute medical, surgical, and rehabilitation services. Sunnybank Private Hospital (SPH) is a vibrant and committed hospital that provides a range of coordinated healthcare services designed to deliver and improve the health and well being of the community it serves. It is situated approximately 13 kilometers south of the Brisbane CBD and is the first Private Hospital north of the Gold Coast.
Selection Criteria:
- Appropriate tertiary qualifications in Social Work
- Eligibility for membership of the Australian Association of Social Workers
- Ability to work autonomously (sole therapist)
- Excellent organisation, time management and self-management skills
- Highly developed communication skills and interpersonal skills
- Ability to work within and provide leadership within a multidisciplinary team
- Commitment to ongoing education and the provision of evidence based practice
- Sound knowledge of local service provision including those through My Aged Care and NDIS
- Knowledge of hospital operations and processes including discharge planning
- Knowledge of EPOA, QCAT, adult guardian requirements and responsibilities
