They are a well-respected organisation that provide specialised care across small boutique Aged Care homes throughout Sydney. There are two opportunities available for Social Workers to operate throughout their homes in Crow's Nest and St Ives, as well as a second opportunity working across the Sutherland Shire.
Position Overview:
We are seeking a highly skilled Social Worker with a minimum of three years of experience. You will play a vital role in providing holistic support to our residents, their families, and the wider care team.
This role will require a specialisation in delivering care for those with dementia, no experience is necessary but will be beneficial - training is provided.
Key Responsibilities:
- Conduct comprehensive assessments of residents' social and emotional needs, with a focus on individuals living with dementia.
- Develop and implement individualised care plans in collaboration with residents, families, and interdisciplinary teams.
- Provide counselling and support to residents and their families to enhance coping strategies and improve overall well-being.
- Facilitate therapeutic group activities and programs tailored to the needs and interests of residents with dementia.
- Advocate for the rights and preferences of residents, ensuring their voices are heard and respected.
- Collaborate with external agencies and community resources to access additional support services as needed.

- Competitive salary package commensurate with experience from $90,000 - $110,000 + superannuation - commensurate with experience level.

Requirements:
- Bachelor's or Master's degree in Social Work or Counselling from an accredited institution.
- Current registration with the Australian Association of Social Workers (AASW).
- Minimum of three years of experience as a qualified Social Worker
- Previous employed experience in supporting a resident and their family through palliation (desirable)
- Excellent communication and interpersonal skills, with the ability to build rapport with residents, families, and colleagues.
- Strong assessment and case management skills, with a person-centered approach to care.
- Ability to work independently and as part of a multidisciplinary team.
- Flexibility to work across homes in Sydney.
