Our Journey
We are looking for an experienced Social Worker (Grade 2) to join our Adult Community Care Team. In this ongoing full time position, you will work Monday to Friday hours at both our new Atherton Road Hub and Clayton PARCS facility. You enjoy a balance of working autonomously and as part of a team, with access to high quality clinical supervision being offered.
The Senior Social Worker of the Clayton AdultMental Health Service will work within a multi-disciplinary team to provide treatment and support to adult clients experiencing Mental Health issues. You will work to address the biopsychosocial needs of individuals on a client centred, recovery based model. Our aim is to delivery high quality care that promotes individual client's self-efficacy, independence and community integration.
A Day in the Life
This role will endeavour to achieve seamless transition for clients and involve their families/carers when they move to Atherton Road Centre for Case Management. While you will provide direct psychosocial interventions to consumers, their families and carers, you are also expected to provide discipline specific support to Case Managers through targeted interventions at Atherton Road and/or secondary consultation where required.
You will enjoy the challenges of working with the complexities of Mental Health to improve individual quality of life and to make a positive difference every day. You will work closely with a range of Mental Health services to effectively co-ordinate client care, including inpatient units, CATT, CCU, PARCS, Mental Health Support, NDIA and Drug and Alcohol services. While the role does not have any case management responsibilities, it will have a discipline specific focus across the Atherton Road Centre. There may also be opportunities for weekend work across these areas on some occasions.
