About the role
This is an exciting new role and a unique opportunity for an experienced Social Worker to provide services to patients whose care is being provided by Hospital in the Home.
HITH manages a range of patients including surgical, medical and speciality medicine. You will be providing care in the patient’s home, via telehealth and within the HITH Clinic at Maroondah Hospital.
The position requires you to be an enthusiastic team player, who is highly resourceful, have strong time management, excellent communication skills, and experience in assessment, interdisciplinary care provision & discharge planning for this patient population.
About you
To be successful in this role you should possess:
- Eligibility for membership of the AASW
- Evidence of current CPE, as per AASW requirements
- Highly developed interpersonal skills
- A strong commitment to high quality care
- The ability to work effectively as part of a diverse team and also work autonomously as required
- Excellent communication and organisational skills
- The ability to lead complex discharge planning
- Conflict resolution skills
- Current Victorian Drivers Licence
