About the Role
An opportunity currently exists for an experienced Grade 2Social Worker to join our team on a permanent basis. To fulfil this role, it is essential that you have experience working in an inpatient hospital social work position. Experience in a sub-acute setting (preferably GEM) would be highly regarded.
You will be required to:
- Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans
- Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes
- Actively participate in quality improvement and research activities
- Provide supervision and participate in clinical education programs as required
- Promote and develop positive relationships within the department, wider organisation and with community partners
Classifications for Grade 2Social Workers range from SC21 - SC24 ($86,730- $99,699 per annum, pro-rata).
Your Contribution
- Bachelor Degree or Qualifying Masters in Social Work
- Significant social work experience in a hospital or related inpatient health care setting is essential
- Current National criminal history check, or willing to obtain
