About the facility
The role is based out of the Torquay and Anglesea Community Health Centres and may include working across other community health sites. The Primary Care teams incorporate multi-disciplinary health professionals from allied health and nursing backgrounds and provides services to the Surf Coast catchment area.
About the role
The successful applicant will be driven to provide collaborative, client centred care to achieve client goals in a range of mediums including face to face and telehealth. The role also requires the successful applicant to contribute to the development of high quality community based services and assist in meeting the key performance indicators.
