The Social Work Service at Monash Health is dedicated to assist patients with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. A passionate and dynamic team assisting patients and families by providing a professional service for patients referred across acute medicine and outpatient clinics. Our Social Work service includes acute and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services.
As a Monash Health Hospital, the Victorian Heart Hospital features a unique, holistic model of care with first ever cardiac services to provide tailored cardiovascular care for all Victorians, and specialised life-saving procedures for our growing community. The first of its kind in the Southern Hemisphere, housing world-ranking Monash University and the Victorian Heart Institute, combining world-class clinical care, research, education and training representing a powerful model of collaboration, innovation and excellence.
About the Role
Excellent opportunity for an experienced Grade 2Social Worker to be part of a dynamic and inspiring social work and broader allied health team working in a collaborative model of care across Monash Health
- Join an active, energetic and friendly Allied Health team.
- Primary based at Victorian Heart Hospital, and at times MMC & Moorabbin Hospitals
- As a Heart Hospital team member support inpatient services & Cardiac Rehabilitation Program
- Work in a setting assisting adult patient cohorts across Cardiology, Cardiac Surgery, Intensive Care, Cardiac Emergency & Ambulatory Cardiac Prevention & Rehabilitation Program, and other specialty areas as required
- A focus on psychosocial assessments, risk assessments, complex discharge planning, bereavement, adjustment to illness and family support.
- Availability one Tuesday per month (1pm – 9pm) to deliver education in the Cardiac Rehabilitation Program.
- Permanent Full Time Grade 2 Social Work position

What you Need
- AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers
- Experience in a healthcare setting is desirable
- Exceptional organisational skills combined with a positive, enthusiastic attitude towards work
- Experience in working with culturally and linguistically diverse communities
- Able to work autonomously as well as collaboratively as part of a wider team
- Well-developed clinical assessment and complex discharge planning skills
- A current Victorian drivers licence
About You
As an accomplished Social Worker, you take pride in working collaboratively with a range of stakeholders including Grade 3, Clinical Grade 4 and Social Work Managers, to provide current evidence-based practice for patients accessing care through the inpatient and community-based setting.
To ensure your success in this role, you will have the following demonstrated skills, experience and knowledge:
- A qualified Social Worker with experience providing a strong focus on patient centred care.
- A strong work ethic and enthusiastic nature, able to adapt to changing settings and shifts during the business week & weekends or public holidays.
- A dynamic Allied Health professional passionate to making positive contribution to patient health
- Outstanding communication & interpersonal skills with unwavering commitment to patient centred care
- Safe best practice, quality care with unwavering commitment to delivering patient centred care
- Highly motivated to be accountable for & manage specific portfolios in our relentless pursuit of excellence
