About the Opportunity
We currently have an exciting opportunity to join the Health Education Interface team (3D Health) team as aSocial Worker, where you will be responsible for:
- Collaborating with multidisciplinary teams and professionals to address the social and emotional needs of students with disabilities and complex health issues in educational settings.
- Providing case management, psychosocial assessments, and therapeutic counselling to clients and their families.
- Independently working with families and professionals in both acute and community settings to implement necessary supports.
- Delivering culturally appropriate, equitable, and high-quality social work services to clients referred to 3D Health throughout South Australia.
About You
To be successful in this role, you will have:
- The appropriate Degree or equivalent qualification, which gives eligibly for full membership of the Australian Association of Social Workers.
- Strong communication, negotiation, and stakeholder engagement skills.
- Skill in various social work interventions such as psychosocial assessments, advocacy, therapeutic support, and case management.
- The ability to coordinate services for clients with complex psychosocial and health needs.
- The capability to work independently, meet deadlines and manage competing priorities.
- A collaborative attitude in multidisciplinary team environments, showing personal initiative and judgment.
