Sydney Local Health District is responsible for the health and wellbeing of more than 740,000 people living within our boundaries, as well as many more from rural and remote parts of NSW and Australia. We have hospitals and Health Services delivered in various locations in the community and in people’s homes.
Where you will be based
Drug Health Services (DHS) aims to reduce the harms associated with substance use for individuals, families and communities and to prevent drug misuse. Services are provided at Canterbury, Concord Repatriation General and Royal Prince Alfred Hospitals and at Canterbury, Marrickville and Redfern Health Centres. Selected programs offer services in community and via approved outreach arrangements.
This position will primarily be based across Redfern Community Health Centre and the Royal Prince Alfred Hospital.
About your new role
The Youth Service provides support and education for young people aged 12 to 25 and their families and or carers in dealing with complex drug and alcohol related challenges.
The core functions of the multidisciplinary community based team are:
- Provide care coordination for young people with drug & alcohol issues, as informed by their professional discipline.
- Facilitate the development of care plans in consultation with all relevant parties utilising harm reduction principles and best practice models.
- Provide referral for young people with drug & alcohol issues to appropriate support services.
- Provide support for other staff (mainstream, drug & alcohol, Youth Sector etc.) in relation to youth drug and alcohol issues.
- Support education and health improvements initiatives for young people experiencing drug and alcohol issues and develop links and partnerships with other services.
The team works in the community setting and aims to provide a safe platform for open dialogue and assists in the development of strategies to empower young people and their families and or carers in their journey towards recovery.
About you...
- Holds a bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers.
- Has advanced clinical knowledge, skill and significant clinical experience in the provision of social work, with experience at Level 2 or higher in the field of Drug and Alcohol and/or Youth Services.
- Holds a current NSW Drivers Licence (P1 and P2 acceptable).
