About the role
The Level 1-2 Social Worker is responsible for providing a professional level of service to the Transitional Aged Care Service CHCK. They are required to undertake case management and social work assessments and plan and implement treatment in line with relevant policies and guidelines.
The TACS Social Worker is expected to be a competent clinical practitioner who is able to work under minimal direct professional supervision and will function as part of the multidisciplinary team.
What you bring
Degree in Social Work and eligibility for membership of AASW.
Current Class C driver’s license.
Demonstrated skills in the provision of social work psychosocial assessment, intervention and case management of older people with complex needs.
Experience working in Aged Care or Rehabilitation with a sound knowledge of community resources.
Well-developed time management skills, excellent written and verbal communication and a high level of computer literacy.
Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
Commitment to continuous service improvement, reflective practice and service development.
Ability to work within the Mission and Values of Calvary Health Care Kogarah.
