Will receive Clinical Supervision, as requirement of the role.About the role:The purpose of CAAT is to provide multidisciplinary integrated care to people experiencing vulnerabilities and complex social and health needs in the community. CAAT aims to ensure clients access appropriate health care to meet their health and psychosocial needs and reduce hospital admissions as first point of treatment.CAAT takes a respectful and consistent approach to engagement and rapport building, utilising colleagues, services and community organisations to provide culturally appropriate, person-centred care.CAAT will provide screening and specialist assessments for vulnerable people with high and complex needs, to ensure more timely and efficient care, reduce hospital presentations and provide diagnostic clarification. CAAT will support appropriate referral pathways to services such as the National Disability Insurance Agency (NDI)), the Aged Care Assessment Team (ACAT), NSW Civil and Administrative Tribunal Guardianship Division (NCAT Guardianship Division), Centrelink for income support and Dept of Community and Justice for housing.Clients of CAAT have complex needs including (but not limited to) mental illness, substance misuse, physical health issues often related to chronic disease, trauma, brain injury, disabilities and experiencing homelessness, or at risk of homelessness.CAAT will provide clients with high quality, evidence-based health care including intervention, assessments and recommendations. CAAT will support existing service providers in identifying gaps in care provision and support existing service providers in appropriate referral pathways, ensuring ongoing care coordination.Service provision is strengths-based, recovery orientated and aligned with the principles of trauma informed care and harm minimisation.Appointments will take place wherever the client feels most comfortable. This can be an inreach meeting in the O’Brien Centre, home visit or outreach setting in the community.Provision of culturally safe, high quality, evidence-based psychosocial and Social Work assessments that identify the clients’ needs, priorities, current risks and protective factors.Development and implementation of interventions through multidisciplinary care plans, risk assessment, brief care coordination, care consultation and identification and exploration of appropriate referral pathways.Provision of evidence-based health care/health promotion and education, including comprehensive Social Work assessment, care planning and referral in order to assist clients to access mainstream and specialist healthcare and support services to improve their health and overall wellbeing.Assessing, planning and evaluating health care and supports in collaboration with clients, case managers, social and informal supports, and the multidisciplinary team.Proactively establishing and maintaining partnerships with specialist service providers and communities to build and strengthen client support networks.Identifying and responding to barriers for people experiencing vulnerabilities and/or with complex health or social needs in accessing services.Active involvement in provision of education, reporting, quality improvement activities and service development.Making recommendations regarding necessary support needs. Completion of resources, support letters, reports or referrals to enable these recommendations.Provision of Social Work assessment if domestic violence is identified by CAAT clinicians as per DVRS (Domestic Violence Routine Screening tool). CAAT Social Worker to comply with Homeless Health Social Worker Response as per HH DVRS Business Rule, SVHNS Domestic and Family Violence Protocol and the NSW Health Policy and Procedures for identifying and responding to domestic violence.The CAAT Social Worker has access to support from the greater Social Work Department, as well as the Homeless Health Social Work team.Diversity and Inclusion at St Vincent's:

Demonstrated commitment to own personal and professional development.7. Computer literacy and demonstrated excellent verbal and written communication skills to ensure constructive and collaborative relationships with a diverse range of stakeholders, throughout various organisational levels.8.
