AddressLismore Community Health has an exciting opportunity to work in a supportive, motivated and professional team providing social work to clients of the Transitional Aged Care Program in the Ballina and Lismore areas. Providing comprehensive and appropriate Social Work assessment and intervention as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD).

If you have/are:
- Relevant tertiary qualifications in Social Work and eligible for membership of The Australian Social Work Association. To be appointed as a Level 2 Health Professional evidence must be provided of three years' clinical experience.
- Ability to provide clinical Social Work services within the scope of practice of the position.
- Knowledge and understanding of person centred care, quality improvement and evidence based practice
- Effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others
