Why this role matters
This role is instrumental in providing outreach case management support to women and children who have experienced and/or are experiencing Family Violence. You will be responsible for delivering high quality and safe Family Violence and child well-being responses to those seeking support, safety and recovery.
About the role
We are seeking a values-driven and proactive Specialist Family Violence Practitioner, at Leongatha, VIC. Reporting to the Team Leader Family Violence.
You will successfully
- Establish effective relationships with clients that encourage self-determination and participation and uphold the principles of the National Family Violence Model of Care
- Provide case management support to clients in accordance with TSA case management models and standards of practice
- Provide ongoing of the client's issues, needs and circumstances from initial intake to exit
- Develop individual case plans in collaboration with clients which include a range of interventions that address the needs/goals of the client
- Refer clients to access holistic support services and establish community connections, and where needed are assisted through advocacy with service providers
This is a Max-Term, Full-Time position, until January 2025. Salary and conditions are in accordance with the SCHADS Social and Community Services Lvl 5.
You will have
- A tertiary qualification in Social Work or related discipline (essential)
- Experience working in the Family Violence sector (desirable)
- Demonstrated case management experience (desirable)
- Highly developed time management and work prioritisation skills with an ability to work autonomously with minimal supervision (required)
