These days are not negotiable.Together, we can do more.Kidney Supportive Care (KSC) involves an interdisciplinary approach that integrates the skills of renal medicine and palliative care to help patients with Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD) to live as well as possible by managing their symptoms and supporting them in living with advanced disease. It is embedded in usual renal care. It also encompasses advance care planning and end-of-life care.The Kidney Supportive CareStaff Specialist will provide specialist outpatient and inpatient consultative support district wide in ISLHD as well as participate in the Renal Service After Hours Roster.We’re looking for a qualified Staff Specialist who:
- Provide high quality, evidence based, patient focused
- Kidney Supportive Care services to admitted and non-admitted patients of ISLHD and their families and carers.
- Ensure a high standard of clinical record documentation is maintained including the completion of patient clinical records (including medication charts/orders) to reflect clinical decisions and changes in the management plan.
- Ensure appropriate arrangements are made for patients on discharge from hospital to maximise continuity of care and good health outcomes, including appropriate communication with medical practitioners external to the organisation including providing detailed discharge summaries as indicated.
- Fulfil responsibilities as mentor for medical students and resident medical staff assigned to the department as required. This includes participation in staff appraisal at regular intervals particularly in relation to registrars and resident medical staff.

- Specialist registration or eligible for specialist registration as a Medical Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).
- Fellowship of the Royal Australasian College of Physicians (FRACP) in Nephrology and/or equivalent specialist recognition. Evidence of prior experience and/or interest in RSC and Palliative Care.
- Evidence of continuing medical education and self-monitoring/audit practices consistent with the scope of practice requirements of the position.
- Proven capacity and/or experience in the provision of effective high quality services, demonstrated through successful clinical practice in an environment appropriate to the requirements of the position including evidence of participation in quality improvement activities relevant to the specialty.
- Demonstrated commitment to teaching with experience in teaching both undergraduate and post graduate students and other staff of the hospital.
- Demonstrate the skills and attitudes appropriate as a Senior Medical Practitioner including modelling a high standard of professional behaviour to junior medical staff.
- Demonstrated ability to work effectively and harmoniously with medical and other health services colleagues as part of a multi-disciplinary health care team and demonstrated ability to communicate effectively with patients, colleagues, and staff and to work as an effective member of the clinical team across multiple sites.
