SalaryWe are looking for Disability Support Worker Superstars to join our growing services working 1:1 with our participants/clients. Shifts vary between 2 hours to 10 hours per day assisting clients in their homes and in the community around the Latrobe Valley & Gippsland.
This role will suit those who are self-motivated, organized and flexible.
As an employee your responsibilities will include but are not limited to:
1. A firm understanding of the NDIS Code of Conduct, Incident Management and Quality and Safeguards practice standards
2. Handling client issues/incident reporting to ensure they are managed in a timely and professional manner
3. Liaising with Choice Support Services team leaders, client plan nominees and client next-of-kin, to gather accurate details for client Care Plans
4. Coordinating with the Accounts & Payroll team, for shift confirmation and adjustments
5. Understanding client NDIS Plans, goals and Care Plans to ensure shift tasks are met
6. Provide personal care, manual handling and medication assistance (as required)
7. Transporting clients to appointments and community access, as per client requirements
8. Supporting a client centric culture and proactive approach
9. Contributing to the provision of relevant information and reporting
10. Contributing to process documentation and continuous improvement activities
11. Providing information to and participating in different care teams as appropriate.
We are seeking highly driven and motivated individuals with strong verbal and written communication and relationship building skills. The right people for our team will possess strong problem-solving skills and exhibit a can-do attitude.
Requirements:-Must have experience in Disability Care and the NDIS, Aged Care or lived experience
Fluent and strong verbal and written communication
Updated First Aid & CPR
Valid Driver's License
Own car with comprehensive insurance

COVID-19 Vaccination Certificate
NDIS Modules Certificate
We are offering casual positions with the possibility of becoming permanent after a probationary period.
