About you
We are seeking a dynamic individual with a passion for Social Support Services and a commitment to promoting independence and well-being. The ideal candidate will have:
- Proven leadership skills with the ability to inspire and motivate team members.
- Experience in planning, delivering, and evaluating Social Support programs, preferably within the healthcare sector.
- Knowledge of CHSP, HACC-PYP, and NDIS frameworks and funding requirements.
- Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, carers, and stakeholders.
- A person-centred care approach, dedicated to meeting the individual needs and interests of clients.
- Demonstrated ability to maintain compliance with quality standards, policies, and procedures.
- A collaborative mindset, with a willingness to work closely with colleagues and external partners to achieve organizational goals.
